People with a severe mental illness are more likely to have poor physical
health such as diabetes or reduced life expectancy Therefore, by providing physical health checks annually we hope to increase their health and wellbeing.
You will support people (aged 18 ) with severe mental illnesses. This will involve
providing physical health checks (training will be provided), developing trusting
relationships with clients providing advice and guidance, and considering other
social factors that may be impacting on their quality of life such as housing
concerns, isolation and employment issues.
This role is part of our Severe Mental Illness health and wellbeing support service
which enables people to access help and improve their health outcomes.
This role requires:
·Experience of working with vulnerable people
·Strong collaborative working skills
·The ability to work both as part of a team and independently
·A good awareness of safeguarding and risk management
·The ability to motivate and influence others
·A driving licence and use of a car (subject to the requirements of the Disability
Discrimination Act).
Contract: Fixed Term until 31st March 2023
Working Pattern: 9am to 5pm Monday to Friday with flexibility required
