We are looking an experienced, motivated and compassionate person to join our established team as a Social Prescribing Care Coordinator specialising in working with patients with cancer. The opportunity is available in Medway and you will work alongside the Medway South Primary Care Network (PCN) team, providing patient coordination and care.

You will support the PCN and practice teams to deliver a high standard of support to patients with cancer. This will involve being in daily contact with patients experiencing a range of cancers and you will be responsible for consulting with them and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, empowering them to be independent whenever possible, and working with the care team to evaluate interventions. You will also support and signpost patients over the telephone, progressing their care pathway in liaison with other colleagues in the PCN and departments in the hospital.

A key aim of Care Coordination is to help patients engage more fully in their own health and wellbeing and you will work in closely with patients, primary care services and hospital team members, supporting cancer care pathways and helping patients feel more empowered and actively engaged in their treatment. Personalised care and the use of shared decision-making techniques support patients to have a voice regarding any treatment undertaken; your role in providing personalised care will involve coordinating interventions, and ensuring records and information are updated and shared effectively to provide the seamless integration of services around cancer patients.

Applicants must have experience of working with people experiencing cancer and cancer care, and be able to support people in a way that inspires trust and confidence. You should have excellent communication skills and be able to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
