Could you help adults with complex needs to access the support they need to live independent and healthier lives? We are looking for someone who is organised, self-motivated and confident, and can work both independently and as part of a team.

You will work alongside multiple GP practices, independently managing a caseload and new referrals, providing personalised support and connecting patients with health and community services to improve independent living, health and wellbeing.

Applicants should have experience of completing initial mental health assessments with provisional diagnosis for mild to moderate depression and anxiety in either adults or children. You should be competent to complete a risk assessment identifying associated risk with diagnosis. As such, it is expected that you will have worked in a similar environment and role.

It is essential that you have an understanding of how to prioritise need, and be able to recognise when to refer on to specialist services, being confident in your ability to communicate at different levels across both the health and social care sectors.

The position is based in Gillingham and is a great opportunity to join our established Social Prescribing team working with Primary Care Networks across the South East.

You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
