This role forms part of an exciting new contract. We are recruiting for a post in Swale, and a post split between Gravesham and Swale

About us: Porchlight is a charity supporting people who are homeless, vulnerable and isolated. At Porchlight, we are passionate about helping vulnerable people to reach their full potential. We strive for a fairer society where vulnerable people find stability, the most excluded are included, and where homelessness and poverty are things of the past.

The role: People with a severe mental illness are more likely to have poor physical health such as diabetes or reduced life expectancy.

You will support people (aged 18+) with severe mental illnesses. This will involve developing trusting relationships with them, providing advice and guidance, encouraging them to undergo physical health checks, and considering other social factors that may be impacting on their quality of life - such as housing concerns, isolation and employment issues.

This role is part of our new Severe mental illness health and wellbeing support service which enables people to access help and improve their health outcomes.

This role requires:
- Experience of working with vulnerable people
- Strong collaborative working skills
- The ability to work both as part of a team and independently
- A good awareness of safeguarding and risk management
- The ability to motivate and influence others
- A driving licence and use of a car (subject to the requirements of the Disability Discrimination Act).
