Hilton Nursing Partners are very proud of our services of "Discharge to Assess" and "Home to Decide" model in the West Kent area We are looking for outstanding carers to join our excellent existing team and to contribute to our Outstanding CQC rated services.

You will be a part of two services, "Discharge to Assess", where you will be working within patient’s homes, proving an assessment service and personal care over a short period of time, to support the patient flow through the hospital. "Home to Decide" working directly with our Registered Nurses in supporting patients who have been identified as needing residential care and supporting them in having an input in their future. Once over the intense period you will support them for the remainder of the 2 week period, while a decision is made as to whether they are safe to stay longer time.

Hilton understands and promotes that recovery is best in your own home. We are not a timed service as person centred care is at the heart of everything that we do. It is important that all our staff members (known as partners) have an input as working as an overall team is key and helps to develop and improve the patient experience.

This is an amazing opportunity to develop yourself further, not only by being a part of this innovated service but also receiving tailored made training by our award-winning Training and Development team.
