About us: Porchlight is a mental health and homelessness charity that supports people in Kent. We're striving for a fairer society where vulnerable people find stability and the most excluded are included. We work with Kent and Medway Clinical Commissioning Group to deliver support that enables people to manage their mental health.

About this role: we’re after a nurse, paramedic or similar healthcare professional (at band 6 level) to work a weekday 9-5 role supporting people in the community.

You will supervise a small team of band 4 equivalents who will carry out NHSE physical health checks on people with mental illness who are being supported at primary care level. This work will take place in the community, people’s homes and some GP practices.

The role will involve:
• empowering people to take charge of their own health and social care support
• ensuring that we’re delivering a quality service to patients, GPs and other organisations
• training staff and ensuring the service is operating to the required clinical and professional standards
• working with the Head of Service to identify ways in which we can build on the work we’re doing
• developing links with local primary care networks.

Porchlight is working with Kent and Medway Clinical Commissioning Group to deliver this service.

What you need:
• Someone who has worked as a community psychiatric nurse (CPN), registered general nurse (RGN), paramedic or in a similar healthcare field at band 6 level
• An empathetic nature and ability to quickly build rapport with people in the community
• Strong clinical skills and willingness to oversee competence in others
• Experience of training and developing a team (useful but not essential)
• Knowledge of Safeguarding for both Adults and Children.
• A driving licence and use of a car (subject to the requirements of the Disability Discrimination Act).
