Are you ready to join our growing team? Social Prescribing offers patients different ways to improve their health and wellbeing, reducing the pressure on health services.

As a Care Coordinator specialising in mental health and based in GP practices, you will provide extra time, capacity and expertise to support patients to understand the conversations they have with primary care professionals.

You will coordinate support and services, ensuring patients do not fall into any gaps while waiting for ongoing support, and support them to access other community or social activities that will aid recovery. A key aim of Care Coordination is to help patients engage more fully in their own health and wellbeing.

You will provide navigation and good quality information to enable patients to make choices and decisions about their care and support needs, bringing this together in a personalised support plan. You will work alongside Social Prescribing colleagues and primary care professionals, and it is essential that you are able to communicate at all levels across both the health and social care sectors.


The position is available in Sheerness and applicants must have experience of working with vulnerable adults and a background in medical or social care where you have delivered mental health support to clients or groups.

You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
